Job Description
The Medical Officer for Home Care Health Services is a specialized contract role (6 months) focused on bridging the gap between hospital care and home recovery. You will be responsible for identifying eligible patients for home-based care programs, managing their clinical transition, and providing ongoing medical oversight in a home setting. This role involves significant coordination with multidisciplinary teams, supervision of locum staff, and a strong commitment to quality improvement and patient safety.
Responsibilities
Patient Enrollment & Transition: Conduct hospital ward rounds to identify eligible patients and apply strict criteria for safe transition to homecare. Participate in multidisciplinary discharge planning.
Clinical Management: Provide clinical oversight for enrolled patients, conducting both scheduled and urgent home visits to manage acute and chronic illnesses.
High-Risk Care: Review and manage high-risk patients, including those requiring PEG feeding, oxygen dependency, or anticoagulation monitoring.
Supervision & Training: Provide clinical supervision and orientation for locum medical officers, ensuring they comply with the approved scope of practice and competencies.
Quality & Safety: Lead Morbidity & Mortality reviews, participate in Root Cause Analyses, and champion patient safety initiatives. Conduct regular chart audits to ensure high standards of documentation.
Improvement Projects: Participate in Departmental and Clinical Quality Improvement Projects (DQIPs and CQIPs).
Education: Provide essential healthcare education to patients and their caregivers to ensure successful home-based recovery.
Requirements
Bachelor of Medicine and Bachelor of Surgery (MBChB) from a recognized institution.
Valid registration and licensure by the Kenya Medical Practitioners and Dentist Board (KMPDB).
Certification in Advanced Cardiac Life Support (ACLS) and/or Advanced Trauma Life Support (ATLS).
A minimum of 2 years post-internship clinical experience in a busy hospital or outpatient environment.
