The Clinical Nurse at the Outreach Clinic T-mall is responsible for providing high-quality patient care, conducting assessments, and implementing nursing interventions within an outpatient setting. The role involves managing patient care plans, administering medications, and collaborating with a multidisciplinary team to ensure clinical excellence and patient safety.
Key Responsibilities
Patient Assessment: Complete head-to-toe assessment and re-assessment of patients to identify their care needs.
Care Planning: Develop written care plans based on patients’ needs, monitor and evaluate effectiveness of interventions.
Monitoring: Carry out hemodynamic monitoring to include vital signs, neurological and fluid status; labs and radiological evaluations.
Documentation: Document all assessment data, investigation results, care given, discussions with the multidisciplinary team, and family/patient interactions in an accurate, comprehensive, and timely manner.
Medication Management: Administer medication safely, monitor patient responses to medication, and report adverse reactions promptly.
Clinical Procedures: Prepare for and assist physicians to perform various procedures in the clinic.
Airway & Pain Management: Maintain adequate airway management (oxygen therapy, nebulization, suctioning) and perform pain assessment to initiate analgesia as per protocol.
Incident Reporting: Promptly report both clinical and non-clinical unusual findings/incidences to the Clinic in charge, Clinical Quality Manager, or Operations Manager.
Patient Education: Identify patient and family education needs and implement appropriate teaching.
Safety & Infection Control: Provide a safe environment for patients through adherence to infection control practices and other safety policies; perform timely safety checks on medical equipment.
Resource Management: Be accountable for judicious and efficient use of resources, including accurate and timely billing of clinic stock drugs and consumables.
Equipment Maintenance: Care for equipment through proper cleaning, storage, per-shift inventory checks, and timely reporting of non-functional equipment.
Discharge Management: Follow discharge procedures for outpatients according to hospital protocol and ensure follow-up appointments are made.
Quality Improvement: Participate in clinic-based nursing education, quality improvement (QI) projects, audits, spot checks, and skill checks.
Requirements
Registered Nurse from a recognized school of nursing or University.
Current licensure with the Nursing Council of Kenya (NCK).
Certification in Basic Life Support (BLS).
Minimum requirement of 3 years’ continuous clinical experience in medical-surgical nursing.
Demonstrated evidence-based knowledge of current practices in general nursing in an outpatient setup.
Demonstrated leadership skills.
